The Wilmington Health Transitions of Care Program allows for one of our expert Nurse Practitioners to conduct home visits within 72 hours of a hospital discharge. Our goal is to ensure that you, your family, and your caregivers understand the reason you were hospitalized, what happened during the hospitalization (including medication changes), and what the plan is going forward.
